Mayo Clinic Sports Performance Daily Covid Symptoms Screening

This must be filled out everyday that you attend training at least TWO HOURS BEFORE your training time.

Off-Season S&C Registration

This is for Off-Season athletes that are currently NOT in a Winter sport. You must register so I can create groups for Monday 1/11.

1. Athletes must bring and wear a mask to all training sessions.

2. Athletes must bring their own water.

3. Athletes must bring CLEAN, DRY athletic shoes.

4. Athletes must fill out the Covid-19 symptoms check-in BEFORE 3pm ON their scheduled lifting day!
